NEUROFUNCTIONAL EVALUATION IN PATIENTS WITH EPILEPSY OF THE LOBE TEMPORARY CANDIDATES FOR SURGERY

Abstract

Epilepsy is a chronic neurological disorder that includes more than 40 clinical syndromes and affects 0.5 to 1% of the world population, initial medical treatment achieves control only 50 to 80% of patients, the remaining 20% ​​is considered medically intractable or drug resistant, of the latter between 5 and 10% may be potential candidates to surgery. Surgical treatment to eliminate seizures is particularly recommended in remediable syndromes surgically, in which epilepsy of the mesial or medial temporal lobe (TME) stands out as the most common form of human epilepsy, the most refractory to pharmacotherapy and in which the best results are reported after resection of the anteromesiotemporal lobe. In 2001, the first controlled study was carried out in patients with epilepsy of the temporal lobe (TLE), which statistically demonstrated the superiority of surgical treatment, in relation to drug, for the evolution profiles of the crises and quality of life, even so, a delay of 15 to 20 years is reported for referral of patients to surgery programs. Therefore, an optimization in the presurgical evaluation is required. that allows the correct identification of the epileptogenic area, since surgical failure, due to incomplete resection or erroneous area of ​​this area represents the main limitation for the development of this therapeutic modality.
